A NOTE FOR CURRENT 6TH GRADE FAMILIES:

Students enrolled in the public schools have a health examination after the last day of 5th grade and submitted to the school prior to entry to grade 7." The physical examination MUST include the starred areas on the form, including the Immunization requirements section, starred on the form.

If your child has not had the required physical exam yet, please download the Health Assessment Record from the clinic web page (aka "CT Blue Form"), and schedule a physical prior to the first day of school. Your physician should also update the immunization section that is included with the form.

If your child has already had his/her physical, please check your records and ask your physician if your student has met the 7th grade immunization requirements. In most cases, it is easy to schedule a quick office visit for an immunization only.

Please submit completed forms prior to the first day of 7th grade. Mail, email and faxes are all accepted.

Postural Screenings (Scoliosis) were conducted by the school nurses beginning January 28th. Per CT guidelines, all 7th grade girls and 8th grade boys were screened. If there was a concern a letter was sent home for you to review with your Primary Care Physician. Please contact us if you have any questions.
