Immunization requirements for Granby Public Schools

                       IMPORTANT INFORMATION FOR Current 6th GRADERS

Section 10-206 of the Connecticut General Statutes requires that “...all students enrolled in the public schools have a health examination after the last day of 5th grade and submitted to the school prior to entry to grade 7."

If your child has not had the required physical exam yet, please download the Health Assessment Record from the clinic web page, or request a copy of the blue form from your school clinic, and schedule a physical. 

**Physicians offices can be very busy in August and Physical appointmentss can be difficult to schedule later in the summer!  

If your child has already had his/her physical, please make sure you have submitted a copy to the Nurse. 

 

The physical examination is a condition of enrollment and MUST include the starred areas on the form:

    including the Immunization requirements section starred on the form.
