Athletic Instructions, forms & fees

If your child would like to participate in a Fall sport at GMHS, please read the information below carefully. This process is applicable to all sports, including football.

The following documents must be printed and brought to the summer clinic hours (see below) with your student:

• Athletic Permission Slip  signed by parent and athlete.

• A completed and signed Athletic Medical Emergency Information Form & Instructions.  This form has to be signed off on by the nurse. For the nurse to sign off on your forms you need a CURRENT PHYSICAL on file with the nurse. A current physical is a physical done within 365 days of participation in the sport. If you do not have a current physical on file, you must complete a Sports Physical Examination Form NO EXCEPTIONS!

• If applicable to student - Authorization to Administer Medication form, Asthma Action Plan ,Severe Allergy Action Plan.

• For any previous injury, a statement of clearance from your doctor.

• Pay for Participation fee: Starting this Fall there will be an option to pay online using ZippSlip, or you may still bring a check made payable to GMHS with your forms. Please keep a look out for the ZippSlip e-mail coming out in July. Please note the fees for B/G Soccer, B/G Cross Country, Field Hockey, and Volleyball is $75. The fee for Football is $350 in which $75 of that goes to the BOE Pay to Participate collection, and $275 is additional Football Pay to Participate funds. Per Board of Education Policy: No student will be excluded from participation on athletic clubs or teams based on inability to pay. Upon receipt of a fee waiver request. The Town of Granby Social Services Department will use established eligibility requirements to determine if individuals or families meet the eligibility requirements. SUMMER CLINIC HOURS

Parents: the nurse will ONLY be available in the summer during the hours listed below. If you are unable to have the Athletic Medical Emergency Information Form signed off by the nurse during these hours, your child cannot participate in practice until the first day of school after getting the form signed off. The Athletic Department IS NOT ABLE to sign off on the Athletic Medical Emergency Information Form, no exceptions.

Please note that Football begins on August 14th and all other Fall sports begin on August 24th. Please see the GMHS Athletics website for coaches e-mail addresses should you have questions regarding information for a specific sport.
