Registration Information

To register a student for Granby Public Schools, the student's parent or guardian must complete and deliver registration materials along with proof of residence to the appropriate school.

Registration Forms:

Kindergarten Registration Letter from Principal & Nurse (for 12-13 school year incoming K only)

Student Registration Form complete online for all students & print

State of CT Health Assessment Record print & complete with ink for all students

Authorization for Administration of Medicine (if medicine will be administered in school) print & complete with ink

Granby Public Schools Health History complete online for all students & print

Transportation Requirements Notice complete online for all students & print

Preschool/Day Care Center Evaluation Form (Kindergarten Registration Only)
