Winter sports will be starting mid November. Students wishing to participate in a team (interscholastic) winter sport must have an Athletic Emergency Information Form completed and signed by the school nurse. Sports physicals are current for 13 months. All students must have a current health physical on file with the nurse, which is dated no greater than 13 months to the date practice starts. Students will be prohibited from practicing or playing come the expiration date if a new physical form has not been submitted. If you have any questions or are unsure when you child’s physical expires please call the clinic and the nurse will be happy to check for you. If they have asthma or require an Epi Pen, they must have medication orders on file with the nurse that allows them to self-administer this medication. Students and coaches do not have access to the clinic after school hours or when on away games.
PARENTS: The Athletic Emergency Information Form is NOT the same as the Health Emergency Form required at the start of school. Athletic Emergency Information Forms can be downloaded from the GMMS website on the Health Clinic webpage under “downloadable forms” or obtained from the Clinic, Athletic Department or coach.
